Everything you see in the picture is included.
The disc has a few small scratches that do not affect gameplay, and the slipcover does have some wear from storage and when it was shipped to me about a year ago. (I had gotten it in a trade towards a spare Metal Gear Solid 3 Subsistence LE... long story.)
