Step 1: Use Cigna as the Insurance. (Using some other insurance providers may require you to entire in a brochure code)

Step 2: Choose the monitor of your choice:Ultra 2 or One Touch Ultra Smart or Ultra Mini

Step 3: Enter your contact information. Where it asks for the BD Monitor number, use: 1-888-232-2737. You don't need a "brochure code" to receive the monitor.

Lastly, it gives you a confirmation: "Thank you for selecting a new OneTouch® Blood Glucose Meter. You will receive your new OneTouch® UltraSmart® Meter and Simple Start™ education materials within 3 business days. "

Hopefully it helps any of you who suffer from diabetes (or know someone who does) and could use a free monitor.

The box includes 10 test strips

Meters generally aren't the problem, its the strips they kill you on.

Yeah, these companies give the meters away is the hopes you'll buy their brand of overpriced test strips for the rest of your life.

I am well aware of the strips killing you and they give them away so you buy their strips. But it is nice to have a spare around

Oh I wasn't disagreeing, in fact I wish I used this brand so I could get another spare.
