It's the little dongle that allows you to use digital audio (or A/V) while using HDMI. I know they were included in Xbox Elite and official 360 HDMI sets.

Im also looking for one if one is available. I know I can hack the cable I have, but Id really like to keep it neat back there and not have an extra 5 cables that arent doing anything. PM me please, thanks!
