The people who design the game are not the same people who keep the game running. It costs a shit ton of money to keep servers running when thousands of people are connected to dozens of servers 24/7. The monthly fee pays for the massive bandwidth and computing power required to keep the game online. Buying expansions and the copy of the game pays the developers to keep making content and give them a return on their investment.
