Did this game ever come in the large box with the dance pad or not?
I know DDRMAX 2 and the later iterations did, but I'm not sure about this one.

No, it was released as game only. And because of that, I think at the time the dance pads were in high demand. Disney Mix and Konamix on PS1 were also released just as game only.
I remember how dissapointed I was with this game when I got it. The arcade version of Max is one of my favorite DDR song lists and since the PS2 version for Japan was a perfect arcade port with every song I had high hopes for the US version. Not 1 of my favorite songs was in the US version, not even the Konami original songs I liked.

I don't really appreciate Konami messing with the original song sets. I never import games anymore (too expensive), so the JP copy was not really an option.
I like having the big box versions of games (GunCon games, DDR games, etc.), so I wanted to know which DDR big boxes I was missing from the set. I found a 3rd party 2-pad set officially approved by Konami with DDRMAX 1 inside, which was really weird. Anyways, I picked it up, but it looks WAAAAAY out of place.
