The say he was shot in the groin, not necessarily the dick. The groin itself is pretty much 75% of the bikini area on the front of your body. Since they didn't go into specifics, such as stating he was actually shot in the penis, I'm guessing the granny hit the bikini area.

Terrible shot, but wonderfully lucky for the drunk. If he was climbing stairs and the granny was shooting downward and aiming at his torso, she had a much, much higher chance of hitting his kidneys, liver, or lungs, and that likely would have ended him.
