They only send out the discs in cardboard sleeves to their rental customers. They keep the case, manual, online pass, etc. in their warehouse. When they sell it, they send you everything that's not the actual disc virtually untouched.
The only bad thing about their used discs is if it's unplayable, they only offer refunds and won't let you do an exchange on the game for another copy. I've only ever gotten one bad game from them, the rest play fine, scratches/chips/dips notwithstanding.
