I think the key here is that the chipped discs almost always work. It's a cosmetic issue, and a pretty minor one. The only way to fix it would be to reinforce their mailers, which would increase the cost of the mailers themselves as well as the shipping costs. Every used game you're buying...

Nope they definitely do it. In the past they've done some pretty large "ban waves" based on some undisclosed criteria. They ban your cc info, address/name, etc too. Not just your account, they won't sell to you personally ever again.
