You'll need City of Villains if you want to access your Super Group's base. If you plan to subscribe to CoH, you should definitely get it anyway since it can be found in most places for $10 and still only one monthly fee.
Has the game changed since launch? Besides new powers, maps, more costumes, epic archetypes, etc., the core gameplay hasn't changed much, though it is more player-friendly now with some updates, such as only getting 50% debt indoors and the addition of exemplars, which are like the opposite of sidekicks. If you quit at level 15 before, you definitely missed out on a lot of the good missions and story arcs, as the game doesn't really hit its stride until around level 30. (Not to mention, you have much better powers at 30 than the two or three attacks and a travel power at 15.) Admittedly, the grind is a bit steep, and debt certainly doesn't help, but CoX has the best community of any online game IMO, and I can't tell you how fun it is when you find a good team or SG.
The game is a bit repetitive, as heroes mainly fight only bad guys and rescue civilians. One new thing about the game is the Safeguard missions, where you tune into your police radio for any crimes happening currently. After you complete a couple of these, you'll get a primary mission to stop a bank robbery. Of course, it's not as fun as the villains' equivalent, Mayhem missions, in which you
rob the bank (and having to break out of jail when you get captured). Generally speaking, villains are more fun to play since they fight heroes and villains alike, allowing for more variety in missions, and even CoV's early maps rival the best designed maps in CoH.
A new feature that had quite a bit of controversy is Enhancement Diversification. That is, you can't slot your powers with too many of the same enhancements, as the improving effects they have on your powers greatly diminish past the second or third.
PvP is pretty fun too. The highest level PvP zone, Recluse's Victory (Lord Recluse is the main villain in this game) actually changes depending on whether heroes or villains are winning. You can watch a trailer of it on Youtube:

If you're looking to start new characters, you should know that each of the 12 servers has its own reputation. For example, Freedom is the most populated server, Virtue is the role-playing server, Liberty is the friendlist server, etc. Currently, there's no way to transfer characters between servers.
Wow, thanks for all of that information. A few questions I have:
What are epic archetypes?
What is each server best known for? (I know you listed a few)
Do you play and if so, which server?
