looking for motorola v265 rebate

As the topic says.. if any body knows where i can find a link to a general pdf file showing this rebate.. it would be greaty apprecitated

its a 50 dollar mail in rebate... I know many promotions are offering this, but I would like to get my phone directly from a verizon store for warrenty reasons.. so if i could pick up this rebate form some where.. it would save me some cash.
