I have this:

I think so, I bought mine used from Hollywood Video and it still had it in there. It was supposed to expire a long time ago, but I tested it out a couple weeks ago and it still worked. But I have Rock Band on 360 so it was kinda useless to me.

Above posts had said it was a "DLC bundle", but it seems to be some PS Home hats, avatars, and a theme. I was expecting it to be some sort of add-on content for All 4 One or something, but it turned out to be pretty sweet! Thanks! ;)
