I've done it before, but I'm not sure if those numbers are unique or not. If so I hope you did yours before you posted this.
 
Yes, those numbers are unique on every receipt. So I think you just forfeited your entry! A lot of the major retail stores and restaurants have these kinds of surveys to get your feedback on your shopping experience, so this isn't a Best Buy exclusive event or anything like that.
 
BB has been doing this a couple years - pretty much every receipt I get has it.

Like CC's surveys, though, I have never seen anyone posting anywhere that they won.
