It's IP restricted for me (British Columbia). I get to the "Please Review and Place Your Order" screen and the "Place your order" button is greyed out with a message along the top that says

"Game Downloads are only available to US customers
We apologize for any inconvenience this may cause you. Please continue shopping on the Amazon.com home page."

Short of going through a proxy I suspect your mileage may vary.

EDIT: It seems so long as you have a US mailing address that it allows you to purchase, no proxy necessary.
