I just got an e-mail with the link to gamesurvey.us

It says they give you survey's that pay $5 a piece and help the video game development teams with game concepts art and stuff like that?

I've yet to actually recieve any checks from them, however some of their surveys are quite interesting. Did one involving a game trailer and you got too record the portions you liked the most.
