It's never explained exactly why Ethan woke up with an Origami figure in his hand, although the blackouts tie back to his son's death. All the newspaper articles about the origami killer may have lead to him making the Origamis in his blackout (maybe because subconsciously he had suspicions that he was killing the boys during his blackouts).
Shelby was investigating in order to clean up any loose ends that could lead back to him. That is why he collected evidence and killed Manfred. If you replay that scene, you will see that the player loses control of Scott for a few seconds and the camera pans to Lauren, which is when Scott makes his move to kill Manfred.

His father knew but was trying to hide it. Gordi Kramer's father put flowers on the grave because he owned the construction site where Scott's brother died.
