Computerbild was giving them away for free a while back (German web site). That is where many of them originated from I am sure.
 
Yes they gave those out also... someone sent me a email with 30 of each so I have been dropping them
(except the Vanquish Tri-Weapon pack)

Some of the more common ones include the "First to PM" or "First to quote this post" rule, or just code jumbles/puzzles/etc to add a degree of effort.
