I'm not seeing any fine print anywhere yet, but the author of the article calls it a "Trojan horse to get people to subscribe to DirecTV" in the comments. I'm a little concerned.
Good point. I suspect more information on the voucher will become available when this anniversary edition becomes available for preorder.
The game is due out on August 27th.
It got pretty mixed reviews in Japan. Praise for relatively unique visuals (the game is essentially fully animated) and an interesting battle system, but it's apparently pretty short and easy.

The game is on indefinite hiatus. NIS is focusing on other projects at the moment (specifically God & Fate Revolution Paradox and Disgaea D2) and put Witch and the 100 Knights on the backburner.
