Expect to wait up to 6-8 weeks to get the rebate, but I've always gotten mine from them and the beauty is they're a CASH rebate in the form of a check, not in-store credit.

Those people got burned because they didn't read the fine print and/or knew it wouldn't work because the form was a fake. The rebate stated that you had to purchase GTA:SA AND the GTA double pack. Many people tried to just submit it for GTA:SA due to an errant form that was circulating around the net. The real form (the one you get from a B&M store) clearly stated that you must purchase both games.

Yes, so they say.

However, I usually use a heavier grade of plain white paper and scan the back of the case through my printer and cut that scanned copy out.

Never been questioned/called on it yet, either.

Just make sure the paper is somehwhat heavy, like the stuff on the insert would be made out of...or else they may catch on.
