It's both, but it's also not as fascist and disruptive as it sounds. Basically if someone complains to the "honor code office" they have to look into it, but they ignore most things unless they are considered "disruptive" to the setting and spirit they strive for on campus.
It's important to note that the entire Honor Code at BYU was originally voted for and designed by the student body back in the 60's. It gets modified from time to time as well.

You are right. The way that girl is dressed in that picture is NOT out of the ordinary for BYU campus. There is much worse that goes ignored by most.
Two things are clear from this story: 1) Whomever handed her that note was a fanatical idiot, like most colleges have in some flavor (think of them as about as common as extreme hippies at other universities) , and most of the student body would not have approved and 2) It's highly likely the note was not meant for how she dressed that day. Very few would consider the way she dressed in that picture immodest on campus.
Suspended from the team for a the rest of the season. He has been reinstated and is currently playing on the team this season and still attending BYU.
Correct. Brandon Davies was suspended. It's unusual for anyone to be kicked out of the school permanently. But if a student does go against the honor code they signed, they can and will be suspended until they "correct" their ways; at which point that can come back.

Special Occasions? Mormons wear that funny underwear all the time. At least, they commit to do so when they get them; if they want to be in good standing in the church.
