You actually get the lesser price item for free. I just did extra meat today and was charged for just the regular one. You can do both if you want, still a good deal.

apparently the system automatically makes the more expensive free. Despite it saying max value is $10, which extra meat and guac would exceed, they'll still give it to you. Nice!
