BOMC2 is supposed to be instant but I got in about a business day. It's 9.95 a month, have to keep it for 3 months, I might stick with it since my hardcover and my girlfriend is a big reader. You can cancel the subscription if you send back your free book within 10 days of receipt, I believe.
IDPromise (e.g Protect My ID) wasn't too hard to cancel, there was a little push to get me to stay.
