At my GameCrazy location, the worker produced a letter from home office that stated 6/26 was the first day they could sell the two together in bundled arrangement.

He thought he could do it this week, but the code doesn't go active until, again, 6/26.

Depends on the store. All of the stores that had it the first week ended it. But Target has it starting today (it's in this week's ad, handy for price matching), and Sears supposedly has it this week as well.
