Question about Myfavefreebies E-mail/Zip Offers

When they say e-mail/Zip offers does that mean that you have to fill in your email and zip code, or email and full address? I know it's a stupid question but I just want to make sure.

I do believe it use to be just email or zip, however now they're suggesting to fill out the second page (roboform works) due to some issues that occured recently.

those who did email/zip offers after 6/15 are getting their points revoked

and she's taking away all email/zip offers
