submitting original UPC code?

I'm trying to get the free blu-rays never submitted one before, confused as of which is the original UPC. I bought one from bestbuy on the back of the box there's a barcode, and on top of the box there's a sticker labeled with UC 1 and 2 bar code lines, do I have to submitt them both?

I think its the one on the movie slip, it should say proof of purchase or check the papers the came with the dvd sometimes they are in there as with some games.
