Where is the $10 Vudu Credit for the Walmart PS3 bundle?

I thought this would have been included as a promo code inside the box, but I didn't find one inside. I got the PS3 bundle from Walmart's Black friday 5 AM sale.

Where is the $10 Vudu credit?

Did you actually buy that PS3 bundle? I didn't get a white paper with a code.

How big was the paper? I only got a Netflix trial paper.
