for in store, it was all blu-ray players including ps3 40 gig (not bundle) and the recently released MGS4 w/ ps3 80 gig bundle.

It's worth noting that those bundle prices are dependent on which game and/or accessories you choose. For example, choosing a Dual Shock controller as your "choose one controller" adds $20 to the price tag. Also there's a limited selection of (older) games to choose from. It's not like you can pick MGS4 as your game.

Also, note that the cheapest one, the Starter Bundle, has different games than the other bundles. Notably, Grand Theft Auto 4. Heavenly Sword. Etc.

No. It's only these online bundles. Even online, the console, by itself, does not include the gift card.
