Under DA:O as a Game Add-On. It's not showing up under Games on Demand, at least on the web version of XBLM.

Yeah it's under Game Add-On, or you can purchase it directly from the downloadable content menu in Origins.

That is the steelbook as the metalpak will not lay flat. Essentially the steelbook has 2 hinges (one on either side of the spine) and the metalpak only has one.
