It's the highlighted area, i.e. the chest/stomach piece and the belt.

I picked up the hadset at gamestop today and asked if they had the codes. I think if it was not printed out on the receipts the guy might have gave me one. I wonder how many were sent out that were extras for gamestops that got the cards.
