Pretty much anywhere, these days. I usually buy mine at Meijer or Kroger - and I have seen them at stores like K-mart, as well.
If they don't have a little display, you can usually buy them at the customer service or at the register aisles.
Just to be clear, these aren't store cards. This is the kind I usually get:

Once you buy one, log onto their site and enter your info (Name, address, etc.) It's been spoken of on this forum in the past that this is a good thing to do, because there's less chance of an offer rejecting the card. (I never used to do this, but do now and have had probably 100% success signing up for things now.)
I've used other ones, and they all work pretty much the same.
You can buy them in different amounts ($25, $50, $100, etc) and they usually charge an extra $3 to $5 when you buy the card. I usually go with the $25 card, myself.
Interestingly, it came in handy today. I left my debit card at home on accident this morning when I left for work (and I never carry credit cards with me) and didn't realize it until I stopped to get a coffee and snack. I remembered I had my gift card on me, so I used that. :lol:
