There's an unbelievable amount of stuff to do and good/neutral/evil all have an effect on what you can/can't do, who helps you/tries to kill you.
Seriously, replay shouldn't be your concern with the game. I've put like 30 hours into it and have barely done the main quests.
my only caveat is that if you don't like running around, exploring, and looting, then you're going to hate the game, 'cause that's half the fun. If you do like that, then great, because it has the most interesting open world of any game I've ever played.
