I used to have that problem all the time in FF, but I never figured out what caused it. I switched to a more convenient platform: online bookmarking services. I just use the links widget in protopage but most use www.murl.com or www.mybookmarks.com
If you use iTunes, Ctrl+Up/Down will change volume. In Media Player Classic, you just need the up and down arrow buttons. Space bar pauses both.
